Questions shops ask
What format do I get?
US Letter. Fillable PDFs open in Acrobat, Preview, and most browser PDFs. Word .docx for shop edits (or upload to Google Docs). Excel formulas open in Excel, Google Sheets, and LibreOffice. Instant download — no shipping.
Can I put my shop name on it?
Yes. The Word files are yours to edit. The PDFs are fillable. FieldForm does not drop your logo for you and does not do custom work. If a line does not match how you run a call, change it.
Is this legal advice or an EPA program?
No. Template only. Not legal advice. Not an EPA or code-compliance program. You and your attorney / AHJ are responsible for use. Observed readings only — no target pressures, no charging calculator, no certification.
What’s the difference between free and $7?
Free is one page: 12 blanks before you open the panel. The $7 zip is the invoice, work order, and full cooling PM checklist — PDF, Word, and the live invoice workbook. No agreement in either of those two.

Refunds?
Digital download. Default is no refund after you have the file. If a charge is a duplicate, or a file will not open, write the platform you bought from. Try Acrobat or Excel first. “It wasn’t a compliance program” is not a refund — that line is on the sheet.
